Step 1: Check you are eligible
To check if you can get a Victorian Carer Card, see Eligibility.
Step 2: Get the evidence you need to confirm you are eligible
Gather the information or documents you need to confirm you are eligible for a Victorian Carer Card.
You can do this in any one of these ways:
- Centrelink Customer Reference Number (CRN)
- Victorian Department of Families, Fairness and Housing (DFFH) vendor number
- Statutory declaration
- Letter from your foster or kinship agency
- Copy of a court order
- An authorised professional completing Section 4 of the Victorian Carer Card application form.

Apply for a Victorian Carer Card
There are 2 ways to apply for a Victorian Carer Card:
- apply online to have your application processed faster
- apply by mail using a hardcopy application form.
Applications are not accepted by email.

Apply online
Complete the Victorian Carer Card online application form.
You can upload any supporting documents with your application. Your supporting documents can be PDFs, PNGs or JPGs. They must not be more than 1 MB. For information on what to use to confirm you are eligible, see Eligibility.
If you want an authorised professional to confirm you are a primary carer, you will need to upload a hardcopy application form. The authorised professional completes and signs Section 4 of the form. You then upload this form with your online application.
You will be given an application number when you submit your online application.
Apply by mail
Applying by mail can take up to 4 weeks longer to process than online applications.
Download a copy of the hardcopy Victorian Carer Card application form (PDF).
If you have trouble downloading the form, you can ask the Victorian Carer Card team to mail you a copy:
- Email CarerCard@dffh.vic.gov.au
- Call 1800 901 958 (between 9am and 2pm), Monday to Friday (excluding public holidays).
Once you have completed the form and included any supporting documents, mail your application to:
- Victorian Carer Card
GPO Box 441
Melbourne VIC 3001.
Note: Do not send us the original versions of your supporting documents. We will not be able to send them back to you.
Incomplete applications
It usually takes around 4 to 6 weeks to process an online application. It can take around 8 to 10 weeks to process a mail application.
If your application is incomplete or incorrect, it will take longer to process.
We may need to contact you to get more information. We may send you an email asking you to fix an error or give us missing information.
